This intake form is completely voluntary. In many cases we understand learning habits and abilities change and develop as a student grows and matures. The best way to allow teachers to accommodate your student(s) is by direct and ongoing conversations. This intake form allows for teachers to prepare beforehand as how to orchestrate their classroom as a whole best. It also allows a doorway for you and the teacher to begin the communication process about your students' needs and growth where and when appropriate.
